Professional Geriatric Care Management

Companion Care's Professional Geriatric Care Managers focus on maintaining the well being, independence and dignity of elders while balancing the special needs and problems of family members caring for them. All of Companion Care’s Care Managers have advanced training, education and experience in gerontology, social work, nursing and/or counseling.

  • An initial consultation that assesses the needs of an individual and provides them with options for individualized care that fit both their lifestyle and their pocketbook
  • On-going management and coordination of in-home care and companion services
  • Preventative care which lessens stress, time and cost to client
  • A single point of access to an entire range of eldercare options
  • Assistance at times of crisis
  • Liaison service for families at a distance
  • Counseling and support

Companion Care is a member of the National Association of Professional Geriatric Care Managers and the American Society of Aging.
